Start with the purpose. Is this column to store computed data, track events, or enable a new feature? Define the type with explicit length or precision. Use consistent naming conventions to avoid ambiguity. Document it in the same place where other schema changes live.

Next, plan the migration. In PostgreSQL and MySQL, adding a nullable column without a default is fast. Adding with a default writes to every row, which can stall queries. If you need a default value, set it after creation with an UPDATE and then ALTER to make it DEFAULT for future inserts.
