The complexity begins after that. On large datasets, adding a new column can trigger a full table rewrite. This can lock the table, block writes, and delay queries. Understanding the storage engine’s behavior is key. In PostgreSQL, adding a nullable column with a default value will rewrite the table in most versions, unless you are on a release that optimizes default-handling. In MySQL’s InnoDB, ALGORITHM=INPLACE can avoid a full rebuild in some cases, but not all.

A safe new column migration often follows a phased approach. First, add the column as nullable with no default. Deploy code that writes to it without reading from it.
