A new column in SQL is more than a single ALTER TABLE statement. On a large table, it can trigger a full rewrite. That means blocked writes, slowed reads, or even outages. PostgreSQL, MySQL, and other relational databases handle this differently, so the migration plan must match the engine. On PostgreSQL, adding a column with a default value can lock the table. On MySQL, depending on the storage engine, it might be instant—unless you have foreign keys or indexes in play.

For production systems, the safest approach is often to add the new column without a default, backfill it in batches, then set the default in a later step. This minimizes locking and reduces the load. Schema versioning tools like Flyway, Liquibase, or built-in migration frameworks can help, but they don’t remove the need to understand the cost of each change.

In distributed setups, schema changes ripple across multiple services. Keep migrations backward-compatible. Deploy application code that can handle both old and new schemas before the change. Then add the column. Then deploy code that depends on it. This three-step process prevents breaking APIs or jobs still expecting the old layout.
