Adding a new column sounds simple. It’s not, when scale is real. Schema changes can lock writes, spike latency, or even take the whole service down if handled carelessly. That’s why every decision—data type, default value, nullability—must be deliberate. At large volumes, even a single ALTER TABLE can ripple through every query plan.
In relational databases like PostgreSQL, MySQL, and MariaDB, adding a new column with a default can rewrite the entire table. For small tables, it’s a second’s work. For big tables, it’s hours of pressure on disk and CPU. The safe pattern for many systems is to first add the column as nullable, then backfill in chunks, then apply constraints. This avoids massive locks while keeping the migration predictable.
In distributed databases like CockroachDB or YugabyteDB, column additions are usually online, but that doesn’t remove the need for planning. Versioned deployment, backward-compatible schema changes, and feature flags make rollouts reversible when issues surface.
